New Patient Information
What To Bring
Keeping your records accurate and up to date is very important to us. In order to ensure the best level of service possible, please make sure to bring the following with you to your appointment:
- Identification card
- Insurance Card: please provide proof of insurance in order to ensure that all billing/payment is made to the appropriate parties
- A list of any allergies or medical conditions: if there is any medication that you are allergic to, or if you have a history of a chronic or medical condition, please let us know
It is advisable to arrive 10-15 minutes early for your first appointment in order to ensure that we receive all of the necessary information from you before your appointment with our doctors.
Upon arrival, we will ask that you please check in with our front desk personnel. They will provide you with a clipboard containing various new patient forms, releases and disclosures. This paperwork is essential in notifying you of your legal rights, how information is used and etc. In turn, our medical office is able to learn more about you and your current health status.
Please answer all of the questions truthfully and as accurately as possible. We know that often times remembering dates can be difficult, but we ask that you please do your best to provide the most up to date information possible.
Our new patient information packet should not take longer than ten to fifteen minutes to complete. Should you have any questions or concerns, please ask one of our friendly staff members.
